As a Elder Care Navigation Law Firm, we offer a holistic solution for families struggling with the demands of an elderly loved one's care. When an elder is no longer able to safely stay at home, the family often has few places to turn to get help. The elder and the primary caregiver are generally alone, dealing with hospitals, doctors, nursing homes, discharge planning, Medicare, Medicaid, VA benefits and more. We have helped hundreds of families with Elder Care Navigation and would like to help you and your loved one as well. If you are helping your aging loved one, you may have the following questions:

  • Can my loved one stay at home?
  • If they cannot stay at home, where will they get the right care?
  • How will my loved one get right care without losing their home and life savings?
  • Will Medicare pay for long-term care?
  • How will Medicaid or VA benefits help pay for care?
  • How can you be sure your loved one is getting the right care?
Components of a Life Care Plan include finances, legal, and heath care advocacy

We created our Elder Care Navigation team to assist you with all of these questions and more. Elder Care Navigation is an integral part of our Elder Law practice. For some Elder Law attorneys, their practice consists primarily of asset protection in the face of nursing home expenses. Our Elder Care Navigation practice, however, does much more than merely protecting the family's assets. Elder Care Navigation recognizes that you and your family need help to navigate the long-term care maze. When you or your loved one is facing imminent long-term care, dementia or other age-related concerns, our Elder Care Navigation practice combines asset protection, estate planning and care coordination to answer all of you and your family's concerns. With a Life Care Plan, you receive the right care sooner, maximum independence for as long as possible, and the ability to age with dignity. Your family gets help finding the right care and services; guidance with legal, health care and long-term care decisions; and peace of mind that comes with knowing you are secure.

Common Questions about Elder Care Navigation

Elder Care Navigation involves helping our elderly clients deal with the legal, financial and healthcare aspects of the aging process. Iit is a holistic solution to help the elderly find, get and pay for good long-term care. Here are some of the common questions we are asked about Elder Care Navigation:

What makes an Elder Care Navigation attorney different from a traditional elder law attorney?

When an elder is no longer able to live at home, an Elder Care Navigation attorney uses an Elder Care Coordinator to help the elder and his or her family obtain good health care. We employ a full-time social worker, Gloria Centonze, whose sole job is to act as a client’s health care advocate. Traditional elder law firms focus solely on asset protection planning while an ECN firm’s primary goal is to get good health care for our clients, while still providing asset protection options if necessary.

What is an Elder Care Coordinator (ECC)?

The role of our ECC is to advocate for our elderly clients to get them the right care in the right place. When your aging loved one has a bad turn in health, such as a fall or stroke, navigating the healthcare system is difficult. The family will have many questions about care, placement, doctors, nursing homes, insurance, assisted living facilities, and much more. Our ECC gives the elder and his or her family support and advocacy in a difficult situation

When would a Life Care Plan make sense for me or my loved one?
